At Community Care of the Lower Cape Fear (CCLCF), we are changing the healthcare experience by changing the way care is delivered. We strongly believe that the best system is rooted in the communities it serves, and we know that efforts directed by doctors and focused on local patients make quality care more efficient and cost-effective. 

We bring over 18 years of experience providing locally-based care management with a proven strategy for health care quality improvement and cost reduction to southeastern North Carolina. We work with Prepaid Health Plans, ACOs, practices and providers to improve patient outcomes, experience, and satisfaction. CCLCF is an independent, 501(c)(3) nonprofit and a National Committee for Quality Assurance (NCQA) accredited organization.

CCLCF was launched in 2003 as an independent 501(c)(3). Based in Wilmington, North Carolina, our geographic footprint spans Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender counties.
