Community Health Improvement Partners (CHIP) is a San Diego–based nonprofit operating in the wellness and fitness services space. The organization pursues long-term solutions to priority health needs through collaboration, community engagement, assessment, education, and advocacy that aim to drive policy, systems, and environmental changes to reduce health disparities. CHIP traces its origins to 1995, when it was formed to meet the requirements of California Senate Bill 697, which mandated community needs assessments by private nonprofit hospitals.

Its work centers on five priorities: reducing and preventing obesity, promoting mental and behavioral health, increasing access to healthcare, improving the social determinants of health, and preventing violence and injury. The organization supports a portfolio of programs and initiatives—such as the Resident Leadership Academy, the San Diego County Childhood Obesity Initiative, Chula Vista's REACH, the Lemon Grove HEAL Zone, a Farm to School Task Force, and related health literacy and public policy efforts—to empower communities and advance system-wide change that sustains healthier outcomes.
