Community Health Network of Connecticut, Inc. (CHNCT) is a not-for-profit managed care organization operating in the health insurance sector. Founded in 1995 by a coalition of Federally Qualified Health Centers, it was created to improve medical services and health outcomes for Medicaid-eligible residents in Connecticut. Based in Wallingford, CHNCT emphasizes a person-centered approach that integrates the whole person into planning and monitoring of care. During its early years, the organization expanded to become the state's largest managed care plan, serving more than 300,000 members. In 2012, the Department of Social Services selected CHNCT as Connecticut’s sole medical administrative services organization, a model that broadened its responsibilities and, under the ASO framework, now serves over 800,000 members.
