Community Healthcare Network is a not-for-profit organization in the Hospitals and Health Care industry that delivers primary and behavioral healthcare, dental, nutrition, wellness, and related support services to New Yorkers. The network operates 13 federally qualified health centers across Brooklyn, the Bronx, Queens, and Manhattan, complemented by two mobile vans that bring health services to underserved communities throughout New York City. It provides care without regard to race, religion, sexual orientation, gender identity, immigration status, or ability to pay, and does not turn people away. Headquartered at 60 Madison Avenue, 5th Floor, New York, NY 10010, the organization employs between 501 and 1,000 people and maintains a website at https://chnnyc.org. In July 2024, CHN launched its own family practice residency program to recruit more primary care providers.
