Our Care Transition Team collaborates with hospitals, rehabilitation and skilled nursing facilities to ensure your return home is as smooth as possible. Then, our team of clinicians work closely with you and your physician to develop an individualized care plan designed to help you reach your optimal health and assure the most positive outcome.

Since no two people are the same, DanielCare Caring Choice conducts a confidential, in-home assessment to gain a better understanding of your (and your family’s) specific needs and expectations. This initial meeting provides you and us with the opportunity to learn about each other and to address any questions you may have about our service.
