Evi-Base is an Encore organization...founded with a driving passion for improving the lives of caregivers and care recipients dealing with chronic conditions. Our North Star is making a profound impact on quality of life and wellness in large and diverse population groups.

We're a collaboration catalyst focused on wide-scale expansion of self-managment education (SME) and long term services and supports (LTSS) into our health systems, managed care organizations, and their patients and members. We are a licensed provider of the Stanford Model for chronic disease self-management and diabetes self-management programs and MaineHealth's A Matter of Balance among others.

We bring a unique skill set to this work and use a different lens to focus on the complexities of meshing community-based, non-profit contractors with the many layers of large healthcare institutions. 

We're here to pave the way for wide-scale, unencumbered access to dependable SME programs because we know there are tens of thousands of Ohioans that need and deserve them.
