Freedom Home Care is a private duty home care agency serving Chicago, the North Shore and Northwest Suburbs. We offers caregiver, companions and certified nursing assistants to help senior citizens with all aspects of personal care, companionship, safety awareness and basic medical care. We conduct a thorough background check and intensive screening of all of our employees. Each is also covered by our liability and workers' compensation coverage. From simple home services to full time nursing care, our staff provides a diverse array of lifestyle assistance and professional care services developed to maximize each client's daily independence, comfort and vitality. We offer a complimentary in-home assessment to develop a custom, individualized care plan to provide only the services that best suit each individual's needs and goals. We are on call 24/7 for emergency situations and have no hourly minimum. Our staff is available day or night, hourly, or full-time to provide services in one's own own. In addition to providing care and services in private residences, Freedom Home Care also provides private care for individuals in independent and assisted living communities, as well as nursing homes, wherever our clients call home. Freedom Home Care is excited to announce our new Client Wellness Tracking System, FreedomCWTS. This system incorporates computer tablet technology and customized software which is placed in the home of each client, allowing caregivers to check in, make notes, and send important updates in real time on their health and wellbeing. The FreedomCWTS system is set to send alerts for falls, weight-loss, change in appetite, change in insulin/oxygen levels and blood pressure readings. Our team is notified immediately if there is a change, and can follow up to prevent hospitalizations.
