The H3C solution targets in-home services with significantly lower associated costs and improved patient outcomes by monitoring patients for specific conditions and health information, providing notifications, alerts, care coordination calls and other communications. We also provide reports to facilitate earlier interventions, encourage compliance with medication regiments, support lifestyle improvements, and reduce the number of hospitalizations, complications, and unnecessary clinical and ER visits.
