Health Care Access Now (HCAN) improves the overall health status of Greater Cincinnati’s most vulnerable residents – those who are likely to have poor health outcomes because they do not have consistent medical care; access to behavioral health care and specialty care; lack support in successfully managing chronic diseases.

Health Care Access Now (HCAN) is a solution to the health disparities and extreme health care costs experienced by an estimated 400,000 residents, 20 percent of our region’s population (adults living in SW Ohio and Northern KY).

Health Care Access Now operates two Care Coordination programs: Adult Health Care Coordination and Maternal & Child Health Care Coordination. Community based Care Coordination pairs individual, diverse needs with the complicated matrix of services in our community. Care Coordination services are provided by a specialized workforce – Community Health Workers. Through advocating, educating, connecting and supporting, CHW’s are partners with clients and the clinical and social services received in the community. 

HCAN delivers an approved CHW Certification Training program and is a leader in developing Community Health Workers in Cincinnati. Our 12 and 14 week training programs develop the skills to provide community based Care Coordination to those most at risk for poor health outcomes.

For anyone needing assistance finding health insurance HCAN has Certified Application Counselors to help navigate the health insurance application process. This service is available free of charge.
