The Home Centered Care Institute (HCCI) is a non-profit organization based in Schaumburg, Illinois that seeks to advance home-based medical care as a central component of healthcare delivery. It supports medically complex patients by enabling primary care to be provided in their homes, through training and mentoring for physicians, advanced practice providers, practice management professionals, academic institutions, and health systems to design and operate integrated, evidence-based house call programs. Its target customers include practices or health systems considering starting a primary care house call program, as well as academic institutions seeking to incorporate this training into curricula. HCCI collaborates with leading academic medical centers, health systems, and industry experts to promote the home-based primary care workforce through education and training, and to develop a research-based model for sustainable program implementation and growth. In September 2024, CareSMS, Inc. announced a partnership with HCCI through the HCCI Connect program.
