Home Nursing Foundation is a Singapore-based health care organization established to deliver cost-effective home-nursing services. It was set up under the Ministry of Health in 1976 and has held status as a voluntary welfare organisation since then. The foundation offers a suite of home-based services, including nursing, medical care, therapy, personal care, and social welfare for patients in their own homes. In addition to its home-care offerings, HNF operates integrated day care facilities known as HNF Wellness Centres in Toa Payoh, in the Central Region of Singapore. These centres run programs such as Maintenance and Dementia Day Care, Community Rehabilitation, Centre-based Nursing, Home Care, and an Active Ageing Hub to address seniors' health and social needs. The organisation emphasizes a person-centered approach designed to help seniors stay active and age with dignity in the community.
