Horizon Care Services provides medical and non medical home care services for those who are in need of private duty nursing or personal care services. Our focus at Horizon Care Services has always been for seniors to stay at home safely for as long as possible. However when health problems are a concern, illnesses develop, or accidents happen, an elderly person may need additional home care services or care in a facility. Our goal as care managers is to coordinate needed healthcare and community services to meet the short-term and long-term care of our clients. Without proper planning and coordination of care, unnecessary expenses and institutionalization may occur.Our care managers assist elderly and disabled individuals and their families with short-term and long-term care needs. Whether family members live near or far, they can be assured that their elderly mother, father, or other loved one is personally respected and cared for properly.The healthcare system can be overwhelming and confusing. Knowing the costs, quality, and availability of healthcare is difficult even for family members who are local. We are familiar with the healthcare and elder care community services in South Florida, Miami, Ft. Lauderdale, and West Palm Beach.
