Founded over 40 years ago as a Federally Qualified Health Center, HRHCare Community Health is one of the nation’s largest community health providers. Excelling in patient care, our network has received the Gold Seal of approval from the Joint Commission, the highest level of Patient Centered Medical Home recognition from the National Committee for Quality Assurance, and is a designated Planetree affiliate.
