Reliance Health Network is an Accountable Care Organization (ACO) committed to providing independent physicians and healthcare professionals with information, support, and resources needed to improve their patients’ care coordination and health outcomes.

As a collaboration of independent, primary care physicians, specialists, and healthcare professionals based in Florida, Reliance is implementing a patient-care model, which is clinically integrated and patient-centered with a focus on prevention, chronic disease management, and enhanced quality of life for our Medicare patients.  Reliance connects healthcare providers with clinics, hospitals and other local resources to provide a community-based approach to patient-care.  Our physician-led organization is committed to the highest quality standards in its development of guidelines for “best practices.”
