Respecting Choices is a system for person-centered decision making that transforms healthcare. Respecting Choices’ goal is to design a system of person-centered care that is guided by the best scientific evidence and is aligned with an individual’s goals and values. This system requires a shared decision-making process that involves respectful interaction among providers, individuals, and families, and keeps the focus of planning on what matters most to each and every person for any healthcare decision. Respecting Choices has over 25 years' evidence of research and practice-based evidence through real-life implementation to better prepare individuals and surrogate decision-makers to make medical decisions (advance care planning) and help clinicians have the skill to align what matters most when decisions need to be made (shared decision making). Through a systematic approach in designing and implementing ACP and SDM processes, the goal of honoring an individual’s healthcare decisions can be achieved.
