Geriatrics primary medical care at home or place of residence.

Senior Care Clinic House Calls' mission is to provide personalized, high-quality care on an as-needed or ongoing preventative basis in the home environment where our seniors live.  We do geriatrics primary care service with a dedicated Nurse Practitioner (NP), in collaboration with our Medical Director. We believe in working with our clients to maintain their level of function in the setting of their own home or place of residence (e.g. Retirement Home, Independent Living, Assisted Living, or Board & Care homes). We manage chronic medical conditions such as hypertension, diabetes, wound, dementia, palliative care,and the like. 

We also believe in sharing knowledge and decision making with our clients, and their designated responsible persons, as we work with them to achieve the best physical state possible at their current level of function. We always consult with the family or responsible person in order to provide informed, consistent care at all times.
