The Center for Comprehensive Health Practice is a multi-disciplinary community health center and substance use program based in East Harlem, New York. It integrates primary care, substance use treatment, behavioral health, and supportive services to address the physical, emotional, and social needs of patients. A team-based approach brings together social workers, pediatricians, nurses, internists, family practitioners, psychiatrists, and other professionals to serve the community. Services are available to anyone regardless of race, ethnicity, gender, sexual orientation, religion, age, or ability to pay, with a focus on prevention and health education. While primarily serving East Harlem and the South Bronx, patients come from all five boroughs, and the organization is accredited by CARF and supported by OASAS and the National Health Services Corp.
