The premise of The Pharmacy Home Project initiative is to create a Pharmacy Home, virtual or otherwise, where drug use information from multiple sources is gathered to better inform prescribing and intervention strategies. The Pharmacy Home Project ultimately aims to promote and encourage medication optimization programs across the spectrum of care that lead to well-coordinated, goal-oriented, continually reinforced drug use plans. Utilizing the power of technology along with better management of patient medications and the availability of better decision support tools, The Pharmacy Home Project will lead to: • improved management of chronic disease, • avoidance of therapeutic duplication, • less prescription drug abuse, • increased use of generics, and • fewer hospital admissions, re-admissions and emergency department visits.
