VNS Westchester is the largest and only independent not-for-profit, Medicare-certified home healthcare agency in Westchester that also serves the Bronx, Dutchess, Putnam, and Rockland Counties.  We help reduce the risk of emergency hospitalizations and the need for other institutional care through our exceptional skilled nursing, rehabilitation, home health aides, social work and care coordination services. VNSW at Home, an affiliate of VNS Westchester, provides certified Home Health Aides on an hourly or live-in basis to help seniors and others in need of assistance with the tasks of daily living. Highly trained and deeply committed, our Home Health Aides provide companionship as well as personal care and household services.

Our recently launched VNSW Community Care Navigation, Inc. provides comprehensive life planning, care navigation and wellness services in the comfort of your home in-person or virtually. Our staff work closely with individuals and/or family members to provide services that take into consideration their goals and wishes regarding physical and cognitive wellness, social and emotional needs, legal matters (e.g., advanced directives), occupational goals, and living environment. Through our Community Care Navigation Program, we can help adults who facing transitional life changes to provide them and their families a greater sense of well being and peace of mind.
