Wellbridge Health is a connected health management company offering a unique high-tech high-touch model of care that significantly lowers healthcare spend by reducing avoidable ER visits and hospitalizations and dramatically improves self-management. Our services include chronic disease management for members of Managed Care Organizations (Medicaid and dual-eligible members) and Accountable Care Organizations (patients of ACO hospitals, systems, and providers); chronic disease management for employees through corporate programs; senior independence aging in place programming through our direct-to-consumer ConnectedForYou program; individuals with intellectual and developmental disabilities (IDD) independence programs; and partnering with clinical research teams to enhance enrollment and broaden diversity. 

The Wellbridge Health model of care management combines simple, customizable 7-day/week remote symptom monitoring (< 2-minutes/day) using an iPhone (that we can provide) with weekly face-to-face video chats with a dedicated health coach -- for over 400 "touchpoints" annually. This comprehensive approach provides our health coaches, who are all master's prepared and have additional training in behavior change theory and techniques, with access to both daily and trend data, to achieve the broad scope of goals that together impact health spend, outcomes and, where relevant, independence:

1) Getting ahead of medical crises; 2) Teaching how to access the right care at the right time at the right place; 3) Improving self-management (including medication management) and preventing disease progression; and 4) Providing resource guidance and removing barriers that address the social determinants of health.
