The placement process for hospital, agency, and geriatric professionals, as well as care givers and patients seeking long term care either as a transition from a hospital, assisted care, or home setting is time consuming.  Complexities of search for an appropriate facility with accurate availability increase with diverse care requirements and payment sources.  Communication between providers is challenged by voice messages left, unanswered emails, open ended requests for information, and stacks of unread faxes.   Difficult placements can be measured in days and weeks, extending to care providers more than 100 miles away.
