Family Care Path develops web-based clinical applications designed to improve patient outcomes, increase physician and clinical process efficiency, and interface with electronic health records. Our core products and services include MyLegacy, a clinical decision support tool for genetically at-risk patient identification; CarePathConnect, telegenetic counseling; and CancerNAV, a patient treatment path navigation tool.  

Providers and payors want a cost-effective means to convey actionable knowledge about health risks to patients in the US and around the world. The value and significance of family health history as an indicator of risk is well documented. However, the ability to collect valuable family health history and the associated disease risks into a patient’s Electronic Medical Record does not currently exist. By integrating the family history tool MyLegacy into the patients’ Electronic Medical Record, we achieve a workflow process which is more readily adopted by Primary Care Physicians, while providing a valuable clinical decision support tool not currently available. 

Additionally, there are only 900 medical geneticists and 4,000 licensed genetic counselors in the US (even fewer in the rest of the world) who have the expertise needed to correctly interpret the data generated by genetic testing. CarePathConnect is an exclusive provider organization of genetic counselors offering genetic counseling to patients who, through completion of MyLegacy, are identified to be at increased risk for certain hereditary conditions. 

Finally, Providers want to deliver high quality and efficient care to their patients, especially in areas with complex care paths such as oncology with numerous medical professionals involved and an extended timeline for patients’ recovery. Family Care Path has developed CancerNav, which has increased navigator efficiency, improved medical team communication and care coordination as well as patient care and satisfaction.
