Health Quality Innovators is an independent, nonprofit health care consultancy based in Richmond, Virginia, with a history of improving care delivery, patient safety, and health outcomes since 1984. It works with hospitals, nursing homes, physician practices, public health agencies, community groups, stakeholder organizations, and patients and families seeking sustainable improvements in how care is delivered. Most of HQI's services are funded by federal and state government agencies, including the Centers for Medicare & Medicaid Services and the Centers for Disease Control and Prevention. 

HQI serves as the Southeast CMS QIN-QIO for eight states—Alabama, Florida, Georgia, Kentucky, Mississippi, North Carolina, South Carolina, and Tennessee. The organization emphasizes reducing avoidable harms, improving population health, advancing health equity, and aligning technology with care delivery. It has been recognized as a Best Places to Work ten times, most recently in 2025, and in 2025 expanded its engagement with CMS through a new contract.
