HRHCare Community Health operates a network of 28 health centers across the Hudson Valley and Long Island, providing primary and preventive care to a broad patient base. Founded more than 40 years ago as a Federally Qualified Health Center, the organization offers services across adult and pediatric care, obstetrics and gynecology, dental, behavioral health, nutrition counseling, HIV care, and health education, along with outreach, translation, transportation, and benefits counseling. Its model emphasizes affordable access to care within a community-centered framework. The network comprises about 350 primary care practitioners and 700 specialists and support staff, delivering care to more than 175,000 people and handling over 650,000 visits annually. It collaborates with local leaders, businesses, social service agencies, housing groups, food banks, and other stakeholders to improve community health, and its work has earned national recognition including the Joint Commission Gold Seal of Approval, the National Committee for Quality Assurance’s Patient-Centered Medical Home designation, and Planetree affiliation. In 2020, HRHCare expanded its COVID-19 response with new testing sites in Brentwood and Riverhead and a partnership with Suffolk County to facilitate testing.
