Founder and owner Forrest L. Preston opened the first facility in 1970 and incorporated the business in 1976. Headquartered in Cleveland, Tennessee, today Life Care employs more than 38,000 associates and operates more than 200 skilled nursing, post-acute, Alzheimer’s, and senior living campuses in 27 states. Two foundational principles were at play, then and now:

The goal of each facility is to be a full partner in the continuum of health-care services in the community in which it operates. Our facilities specialize in skilled nursing, post-acute care, as well as inpatient and outpatient therapy in well-equipped, on-site rehabilitation gyms. Additionally, many of our centers provide Alzheimer’s and dementia care.
