The MOH Office for Healthcare Transformation (MOHT) is the Ministry of Health's innovation office, established in 2018 to drive system-level transformation in healthcare. We work at the intersection of policy, operations, and technology to develop and scale care models that empower health, improve outcomes, and deliver better value for Singaporeans.

Our mission is to help reshape the health system, so it keeps people healthier for longer. We partner closely with MOH, public healthcare clusters, and community stakeholders to co-design, iterate and test solutions, so that they are ready for real-world deployment and sustainable mainstream adoption.

MOHT’s work is focused on three priorities:
- Primary Care Transformation – Equipping primary care providers to strengthen care relationships, extend reach, and deliver value-based primary care
- Mental Health Innovations – Enabling tiered mental health care in a data-informed way, despite workforce constraints 
- Frailty Management – Detecting and managing frailty earlier to preserve function, independence, and equity
