NJII Healthcare operates within the New Jersey Innovation Institute and is based in Newark, New Jersey. It aims to improve healthcare delivery through its iLab, which hosts initiatives such as the Garden Practice Transformation Network that supports clinicians in moving from fee-for-service toward value-based care under a CMS program. The iLab also leads the New Jersey Health Information Network, a statewide data-sharing platform developed with the New Jersey Department of Health, enabling connection among health information exchanges, hospitals, and clinicians for data such as admissions, discharges, transfers, and immunization records, anchored by a Master Person Index that uniquely identifies patients across the continuum. Through NJHIN Use Cases, it enables more than 500 providers and over 20 long-term and post-acute care facilities to access state public health registries and other data sharing sources to support population health and improved clinical outcomes.
