PrimeWest Health is a health coverage partnership formed by 24 rural counties in Minnesota to serve individuals eligible for Minnesota Health Care Programs. It currently offers coverage to more than 45,000 residents across 13 Minnesota counties, with eleven additional counties joining the partnership in 2020, though coverage was not provided in those counties that year. The organization’s provider network comprises more than 12,600 medical and social services providers spanning Minnesota and neighboring states, including North Dakota, South Dakota, Iowa, and Wisconsin.

Based in Alexandria, Minnesota, PrimeWest Health maintains a sizable workforce and operates as a full-service health plan with responsibilities that include enrollment and member services, care coordination and quality management, provider network management, claims processing, information systems, and regulatory compliance with state and federal authorities such as MDH, DHS, CMS, and NCQA. In late 2025, PrimeWest Health announced a partnership with Itiliti Health to modernize its prior authorization process.
