Sapphire Care Management, LLC., is a group of over 80 healthcare professionals established on 2012. We are dedicated to the delivery of high-quality clinical services for various healthcare constituents including Medicare Advantage plans, hospitals, physicians and medical groups. Our clinical faculty are fully credentialed according to Puerto Rican, federal and Medicare standards, and include physicians, nurses, social workers, and nutritionists.

We developed and operate bilingual call centers for providers or patients customized to our clients’ needs which comply with all Centers for Medicare and Medicaid Services (CMS) regulations. 

The company enjoys a long history of successful implementation of projects in Puerto Rico including the first private cardiac surgery program on the island, outpatient surgery centers and multiple Medicare Advantage programs described further under tab Services.
