Mission Ready Software has the largest database of software failures analyzed by root cause.  We also have the largest benchmarking study of 600+ factors that lead to technical debt and escaped defects.  From these, we developed methods to predict software defects and failures before the code is even written.  Other prediction models cannot be used until later in the development process, when it's too late for the models to affect decisions or design.  These models have been used in industry for more than 20 years and are available in training, software or consulting services. We wrote the Common Defect Enumeration (CDE) on the DoD R&M CoP website.  Mission Ready Software has authored one of the only industry guidebooks on software FMEA.  We have identified more than 400 software failure modes/root causes that apply to virtually all types of software systems. These are also available in training, software and consulting services.
